NOMINATION FORM

Nominate a Catholic High School Athlete, Coach, or Team to the 2012 All Star Catholic High School Sports Faith Hall of Fame by filling in the information below. A nominator is the person or school recommending a candidate for induction.

You may nominate yourself, your team(s), coach(es), or other worthy candidates.
There is no limit on the number of candidates you may nominate: however, you must fill out one form for each candidate you are recommending.
There is a $9.95 optional donation to offset the cost of processing each application. A portion of this fee will be remitted to the educational, religious or other charitable institution that referred you to the competition. If you would like this donation to be made, please provide us the information in the space below. Whether we recieve the donation does not affect the ability of the athlete, coach or team to be selected as the winning inductees.
Candidates (Nominees) under the age of 18 will be required to provide us a parental release form as a condition for certification as applicants to the 2012 All Star Catholic High School Hall of Fame.

What Happens Next? The Candidate you are nominating will receive an email notification of your recommendation once your nomination form below has been submitted and processed. You will also receive an email notification when the submission process has been accepted, and when the candidate has completed the application process.

NOMINATE AND WIN: Each time you nominate a candidate for the 2012 All Star Catholic High School Sports Faith Hall of Fame, your name will be entered into a drawing to win sports memorabilia, and tickets for two to the Sports Faith Hall of Fame Induction Ceremony at Chicago Bears Halas Hall. Odds of winning will vary according to level of participation.
